This is a useful little book for recording a family’s medical history; it is spiralbound for convenience. Part One has forms for past health records, including immunizations and allergies, physical examinations and tests. Part two contains a list of questions to ask doctors, surgeons, and druggists, as well as checklists of symptoms, travellers’ records, and visits to your doctor. Part three has material on definitions, tips for visits to the doctor, and medical expense records. This kind of material is very valuable; it is a good idea for everyone to have some kind of a medical history in his or her possession (especially when travelling or relocating).
